مراقبت و درمان زنان باردار ی که بیماری تشنج دارند چالشهای جدی را در زمینه اتنخاب و تجویز دارو در پرتو اطلاعات مربوط به تاثیر ان داروها رویزمینه جنین و بارداری اعم از بارداریهای برنامه ریزی شده و یا غیر ان و نیز واکنش های  پیچیده بین داروها و اورمونهای زنانه رودر روی طبیب و بیمار قرار می دهد.

مطالعات بالینی انجام شده در زنان باردار اطلاعات ذیقیمتی را در واکنشهای متقابل پیچیده میان هورمونهای استروییدی سیکل ماهانه-دفعات تشنج-داروهای صد صرع-داروهای ضدبارداری و اختلالات اعصاب و غدد داخلی(نورواندوکرین) یدست می دهد.

ارتباط سه جانبه بین صرع داروهای ضد صرع و اورمونهای استرویید زنانه:

اورمونهای حنسی استرویید زنانه به عنوان نورواسترویید مستقل عمل می کنند و مطالعات علمی در جانوران نشان می دهد که انها محرک سلولهای عصبی بشمار می روند و برعکس پروژسترون واجد اختصاصات بازدارنده نورونهای سلسله اعصاب  عمدتا از طریق متابلیت خود الوپرگنانولون محسوب می شود.انها این تاثرات را طریق واکنشهای سریع غشا نورونها ویا از طریق کندتر ژنومی اعمال می کنند. به این ترتیب می توان بسادگی متوجه شد که تغییرات در سطح سرمی اورمونهای استرویید زنانه در کنترل تشنج نقش جدی و مهمی دارند.

تغییرات در میزان استرادیول کلیرانس برخی داروهای ضد تشنج را افزایش داده و به کاهش میزان سطح سرمی ان داروها منجر شده و در نتیجه می تواند بیماری را از کنترل خارج کند.

از سویی هم داروهای ضد تشنج و هم خود بیماری تشنج قادر به تغییرات در محور هیپوتالاموس-هیپوفیز-تخمدانها بوده و در نتیجه اثرگذاری در سیر درمانی بیماری تشنج شوند.

 

 

محور هیپوتالاموس-هثپوفیز-تخمدان

تشنج کاتامنیال :به تشنجی اطلاق می شود که اه نوعی به یکی از مراحل دوره عادت ماهانه مربوط می شود با هرکدام از انواع تشنج ها می تواند همراه باشد ولی بیشترین گزارش با تشنج های موضعی است (فوکال)و یک سیکل قاعدگی معمولا 28 روز است که روز 1 روز اول ان و روز 14 روز تخمک گذاری بوده که از روز اول تا روز 14 فاز فولیکولار از فاز های دوگانه یک سیکل قاعدگی و از روز 14 تا روز 1 بعدی را فاز لوتئال نامیده که فاز دوم بشمار می رود.تشنج در ایام قبل و بعد از مراحل اغازین فازهای اول و دوم می تواند تشدید و رخ دهد ونیز در انهایی که فاز لوتئال کافی نداشته ودر نتیجه تخمگ گذاری هم اتاق نیافتاده و زمینه را برای وقوع تشنج براثر اکاهش پروژسترون فراهم می کند.صرع قاعدگی بیشتر زمانی رخ می دهد که میزان استروژن بیشتر از پروژسترون بوده و یا انکه میزان ان سریعا تغییر کند.

در زنانی که تشنج دارند تقریبا 12تا 75 در صد در ردیف صرع ایام عادت ماهانه قلمداد می شوند و بر پایه امارهای دیگر احتمال تشنج در زنانی که این کسالت را دارند در فازهای مختلف بیماری دوبرابر می شودو رویهمرفته در زنانی که تشنج فوکال دارند حدود 1/3 تشنج کاتامنیال دارند.
